Privacy of Personal Information

COLLECTION OF PERSONAL INFORMATION

To access features of MyEAPConnect, you must first complete a brief registration form. During registration you are required to provide certain personal information (such as name, email, and employment details). By registering, you agree to receive certain communications from the UAB Employee Assistance & Counseling Center. After registration, you may be given the option as to whether or not you want to receive other types of communication from us.

We collect personal information about you, including personal information that you provide when you register to use the Site (as described above) and information collected during your employment with UAB Employee Assistance & Counseling Center. Providing this initial information is necessary to participate in the MyEAPConnect features or services, to authenticate your identity, and to tailor the services to help meet your needs.

We may also collect certain information about you but does not identify you and information about your internet connection, the equipment used to access MyEAPConnect, and certain MyEAPConnect usage data.

As part of the MyEAPConnect services, we may collect sensitive information related to your mental health, well-being, or other personal matters that you voluntarily provide.

We collect information directly from you when you provide it to us or automatically as you interact with MyEAPConnect.

USE OF PERSONAL INFORMATION

Personal information collected through our Site is used by us to, among other things:

·         process and manage registrations,

·         operate and improve the MyEAPConnect services,

·         track the Site’s usage policies and statistics,

·         personalize your experience and provide resources tailored to your needs,

·         make the Site or services offered through the Site easier to use by eliminating the need for you to repeatedly enter the same information,

·         receiving electronic messages,

·         making or cancelling appointments,

·         completing and signing pre-visit forms,

·         monitor and analyze usage trends to improve the MyEAPConnect services,

·         comply with legal obligations,

·         protect our rights,

·         for any other purposes to which you consent.

We also use your personal information to communicate with you in other ways. For instance, when you register on this site, we may use your personal information to send you an email confirming your registration. We also use personal information to inform you of significant changes to this Privacy Statement.

DISCLOSURE OF PERSONAL INFORMATION

As part of our commitment to supporting the well-being of our employees, we have partnered withHarting EAP (the Provider) to provide the MyEAPConnect services. Personal information that you provide in connection with MyEAPConnect, including any sensitive data related to your well-being, will be stored on the servers of our third-party EAP service provider. By using the MyEAPConnect services, you acknowledge that your personal information will be stored on the provider’s servers. This storage is essential to provide you with access to MyEAPConnect.

We will provide your personal information to other companies with whom we have signed agreements to protect your information.  These companies provide services on our behalf, such as creating your account, handling the processing and delivery of communications, hosting the Site, and delivery of content and services provided by the Site.

We and the Provider may access, use or disclose your personal information for other reasons, as permitted by applicable laws, including if we believe such action is necessary to comply with the law or legal process, protect the personal safety of users of our website or members of the general public, or enforce our Terms of Use.

We may also share your personal information with other third parties if you provide your express written consent.

[UAB Employee Assistance & Counseling Center does not obtain detailed information about your use of MyEAPConnect unless explicitly authorized by you or required by law. However, we may obtain and use aggregate or de-identified data that does not personally identify you without restriction. 

ACCESS TO PERSONAL INFORMATION

You may have the right to request access to, and correct, the personal information collected by the MyEAPConnect services. To update or change the personal information you have provided the UAB Employee Assistance & Counseling Center , or to unsubscribe from MyEAPConnect, please contact the support email on the MyEAPConnect site.

Security

We have implemented procedures to help protect the personal information that you provide to us. However, no method of transmitting or storing electronic data is ever completely secure, and we cannot guarantee that such information will never be accessed, used, or released in a manner that is inconsistent with this Privacy Statement.  If such improper access, use, or release of your personal information occurs, we will comply with all applicable laws in addressing the improper access, use, or release of your personal information.

Data Retention

We will retain your personal information for as long as necessary to fulfill the purposes for collecting it or as required by applicable law. Once your information is no longer needed, we will take steps to securely delete or anonymize it.

Children Under the Age of 14

We do not knowingly collect personal information from children under the age of 14. No one under the age of 14 may provide any information to or on the MyEAPConnect services. Should we learn that a child under 14 has provided us with personal information we will immediately delete that information from our database.

Our Practices with Respect to Non-Personal Information

We may collect other, non-personal information about your visit, including the pages you view, the links you click, and other actions taken in connection with your use of the Site. We may use “cookies” (a small, unique identifier text file) to gather non-personal information. We view non-personal information as non-confidential, and we may use or disclose non-personal information without restriction or obligation to you. However, we may combine non-personal information with other information, including personal information we have collected about you. If we do combine non-personal information with personal information in this way, we treat the aggregate information in accordance with the practices for personal information that are described in this Privacy Statement.

We are committed to ensuring that your use of MyEAPConnect remains confidential and free from monitoring or tracking by the UAB Employee Assistance & Counseling Center . To this end, we expressly prohibit the use of web beacons, pixels, or any other tracking technologies by our organization in relation to your access and use of MyEAPConnect.

Changes to This Privacy Statement

We may update this Privacy Statement from time to time. We will notify you about material changes to this Privacy Statement by sending a notice to the e-mail address you provided to us or by placing a prominent notice on the Site. Any changes to this Privacy Statement will be effective immediately upon posting on our website or within the MyEAPConnect software. Your continued use of MyEAPConnect following the posting of a revised Privacy Statement constitutes your acceptance of the changes.
